The Health Resources and Services Administration (HRSA) estimates the U.S. will face a shortage of 124,000+ full-time-equivalent physicians by 2027. For international medical graduates (IMGs), that number shows the size of the opportunity in the U.S. healthcare system.
For IMGs looking at the U.S. system, those numbers really put the sheer scale of the opportunity into perspective.
Getting through the journey from abroad takes careful planning, but breaking it down into distinct phases makes the process much more manageable.
What Is USMLE?
The United States Medical Licensing Examination (USMLE) is an examination pathway used as part of the physician licensing process in the U.S.
If you are an IMG, you need to clear both Step 1 and Step 2 CK to meet the Educational Commission for Foreign Medical Graduates(ECFMG) Certification requirement. While Step 1 tests your basic sciences, Step 2 CK dives into how you actually apply clinical knowledge to patient care. Step 3, which comes much later, cannot be taken until you have your ECFMG certification and have already passed the first two exams.
What Are the USMLE Steps for IMGs?
Navigating the USMLE as an IMG boils down to a structured journey toward ECFMG Certification and, ultimately, landing a residency spot.

Check USMLE Eligibility
Before you even think about cracking open a textbook, you need to make sure your medical school clears the ECFMG’s checklist. Head over to the World Directory of Medical Schools (WDOMS) and confirm your school is listed with an active ECFMG Sponsor Note that covers your specific graduation year. You’ll also need to show credit for at least four academic years of medical school.
Take USMLE Step 1 and Step 2 CK
Once that’s done, the examination phase kicks off with Step 1 and Step 2 Clinical Knowledge (CK).
Step 1 dives deep into the foundational medical sciences and is reported as pass/fail, while Step 2 CK shifts gears to test how well you actually apply clinical knowledge to patient care and provides you with a three-digit score.
For IMGs, a strong Step 2 CK score is an important part of the residency application. Roughly 85% of the weightage is given to it.
Passing both is mandatory to continue with your ECFMG Certification.
Complete ECFMG Certification Requirements
Passing Step 1 and Step 2 CK does not by itself complete ECFMG Certification.
You need to fulfil the clinical and communication skills requirements through an applicable ECFMG Pathway. You must also obtain a satisfactory OET Medicine result, regardless of your native language, citizenship, or language of medical education.
The current OET requirements are:
- Listening: 350
- Reading: 350
- Speaking: 350
- Writing: 300
All four scores must be achieved in a single OET Medicine administration.
If you’re aiming for the 2027 Match:
Submit your Pathways application by January 31, 2027.
To avoid any processing delays with your OET score, ECFMG strongly suggests sitting for the exam no later than the final test dates in December 2026.
Also, a quick procedural heads-up: since January 2026, international students and grads have been routing their USMLE Step 1 and Step 2 CK registrations through the FSMB, even though ECFMG is still running the show for overall certification eligibility.
Build and Submit Your Residency Application
Once that piece is locked in, you’ll shift gears to Electronic Residency Application Service (ERAS) to send out your residency applications.
The portal opens up for submissions on September 2, 2026, and programs get their first look starting September 23. This is your main window to pull together everything you’ve built- your CV, personal statement, clinical experiences, research background, publications, and solid letters of recommendation.
Register for the Match
Note that the ERAS and the National Resident Matching Program (NRMP) are two completely separate tracks. You’ll need to register for the NRMP Main Residency Match independently, and eventually put together and certify your Rank Order List (ROL).
Keep these key dates on your radar for the 2027 match:
- September 15, 2026: Applicant registration opens
- February 1, 2027: Ranking opens
- March 3, 2027: ROL certification and ECFMG verification deadline
- March 15, 2027: Match status is released
Residency and Beyond
After interviews, you submit your preferred programs through your Rank Order List. The Match determines where you will begin U.S. residency training. After residency, you can do a fellowship to get some more training.
