Documents Required

Required documents under the Scheme for Provision of Shelter House for Looking after the Animals

  1. Application in prescribed proforma (copy enclosed)
  2. Details of Beneficiaries – Facilities available with the AWO.
  3. Details of Staff Employed.
  4. Detailed proposal and its justification for construction of the shelter house;
  5. Authenticated copy of the Registration Certificate in respect of the organization (certified by the Registrar);
  6. A copy of the Memorandum of Association;
  7. Blue Print of construction plan duly approved by the local authorities (Municipal Corporation/Municipality/Gram Panchayat) with survey number, Village, Tehsil and District of site address.
  8. Lease/gift/sale deed should be registered by the Registrar. Lease deed should be for a period of minimum 30 years;
  9. Attested copy of Revenue Record of land showing its title in the name of organization from the Tahsildar.
  10. A certificate from the concerned Registrar/Sub Registrar along with documentary proof that the land on which shelter house is proposed to be constructed is in the name of the organization and without any encumbrances;

  11. The estimate showing the amount of financial assistance required for construction of shelter house in relevant break-ups worked out and authenticated by an approved valuer;
  12. A certificate from the State Government Approved Valuer to the effect that the rates quoted for the construction of the proposed Shelter House are not more than prevailing PWD/CPWD rates;
  13. List of governing body;
  14. Latest audited statement of accounts duly certified by the Chartered Accountant, in respect of the organization for the last financial year, in the under mentioned parts:-
  15. (i) Balance Sheet (ii) Income & Expenditure Statement (iii) Receipt & Payment Statement and (iv) Audit Report;
  16. Details of Animal Welfare activities performed so far and expenditure incurred for this purpose;
  17. Details of financial resources to run the proposed shelter house smoothly and area to be covered for rescue of animals;
  18. Details as to whether the organization has ever received any grant-in-aid from the Ministry or AWBI and if so, please indicate whether the Utilisation Certificate of that grant has been submitted or not;
  19. Quotation in support of cost of medicines/medical equipments required;
  20. Details of existing covered/constructed area, its capacity and proposed to be covered;
  21. Copy of Memorandum of Understanding/Agreement entered into with concerned local bodies (i.e. Municipal Corporation, Panchayat etc.) to the effect that the organization will accept stray animals from them to the extent of sheltering capacity for which the grant is sought;
  22. Recognition form for getting recognition from the Board, if not recognized so far (copy enclosed).
  23. All original documents in regional language should be furnished with an attested translated version in English/Hindi by the Notary Public.

How to apply for ABC Grant

Documents Required

AWOs under ABC:

The following documents marked (ü) under ABC scheme should be sent as annexure in support of your proposal: -

  1. Application in prescribed proforma
  2. Detailed proposal and its justifications alongwith gist of activities undertaken and assets required;
  3. Details of area to be covered for this programme; total dog population in this area duly certified by the concerned municipal corporation (s); method adopted for carrying out dog census and how & in what time the organization will complete the programme in this area;
  4. Photocopy of Registration;
  5. Memorandum of Association showing ‘animal welfare’ one of its objectives;
  6. The estimated cost of proposed immunization and sterilization of dogs along with number of dogs and the area to be covered;
  7. List of Governing Body/ Monitory Committee Member list;
  8. Audited Accounts of last three years duly certified by a Chartered Account i.e., (i) Audit Report, (ii) Balance Sheet, (iii) Receipt & Payment Account and (iv) Income & Expenditure Account;
  9. Details of Infrastructure viz. Operation Theatre, Kennels (with their capacity), Vet. Surgeon, Vehicle, man-power etc., available to implement this project;
  10. Details of activities undertaken in the field of Animal Welfare both in physical and financial terms during last three years;
  11. Details of financial assistance, if any, received from any other agency for this purpose;
  12. Details of collaborative arrangement that exists between the concerned Municipal Corporation (s) and the NGO to implement the project and a copy of Memorandum of Understanding entered into with every concerned municipal corporation;
  13. Furnish the photocopies of the Animal intake/out put register daily basis (date wise) duly signed by the Commissioner.
  14. Furnish the surgery register daily basis (date wise) duly signed by the local body official & Vety.Surgeon/Vety.Doctor with registration number.

The following documents for ABC programme is required by the Board.

Local Body:

  1. The Municipality if collaborated with any AWO/NGO for carrying out the ABC operations; an Original document of Memorandum of Understanding (MoU) duly signed by the Municipality & AWO to be furnished.
  2. Furnish the infrastructure/facilities availability in the Municipality/AWO (Shelter, Dispensary, Vety.Surgeon, Para Vets, Vety.Staff, animal handlers, medical equipment, Autoclave, supply of dogs, dog catcher, dog catching/relocating, existing system of drainage, ambulance etc.,
  3. Furnish the photocopies of the Animal intake/out put register daily basis (date wise) duly signed by the Commissioner.
  4. Furnish the surgery register daily basis (date wise) duly signed by the local body official/Vety.Surgeon/Vety.Doctor with registration number..
  5. Detailed proposal and its justification along with gist of activities undertaken and assets required.
  6. Details of area to be covered for this programme; total dog population in this area duly certified by the concerned Municipal corporation (s); method adopted for carrying out dog census and how & in what time the organization will complete the programme in this area;
  7. Details of financial assistance, if any, received from any other agency for this purpose
  8. Furnish the Acceptance of 50% fund borne by the Municipality.
  9. Furnish the Managing Monitory committee member list.
  10. Furnish the photographs and photocopy of the activities of the AWO of ABC maintained separately by the municipality.
  11. Furnish the Sterilization Certificate with month wise breakup detail for ABC operation duly certified by the local body officials.

Scheme for Ambulance Services for Animals in Distress - supporting documents

  1. Application in prescribed proforma;
  2.  Details of Beneficiaries – Facilities available with the AWO – Annexure I.
  3. Details of Staff Employed- Annexure II.
  4. Detailed proposal and its justifications alongwith gist of activities undertaken and assets required.
  5. Photocopy of Registration.
  6. Memorandum of Association showing ‘animal welfare’ one of its objectives duly certified by the Gazetted Officer (or) Notary Public, if the organization is not recognized by the Board.
  7. Type and estimate of proposed ambulance.
  8. Proforma voucher from the supplier of the ambulance vehicle mentioning the cost of the vehicle.
  9. Challan form / estimate from a dealer in support of the cost of the equipments/modifications in the vehicle.
  10. List of Governing Body.
  11. Audited Accounts of last three years duly certified by a Chartered Account i.e.,
  12. 1.Audit Report, (b) Balance Sheet, (c) Receipt & Payment Account and (d) Income & Expenditure Account.
  13. Details of financial assistance, if any, received from any other agency for this purpose.
  14. Details of Veterinary Doctor/Para Vety. name, full address, Veterinary Council Registration number and full time or part time & assistance staff with AWO
  15. Details record for last three years i.e. how many treated and rescued animals and certified by the Veterinarians. (As per enclosed proforma)
  16. 15. How many emergency calls were attended last three years. 16.Details record of the rescued of illegal slaughter house animals in last 3 years.
  17. If the organizations does not own the vehicle/ambulance, how does it pick up/rescue the animals.
  18. If the AWO already have Ambulance, the details of the vehicle whether purchased from MEF/AWBI grants, when purchased, Number of Kms. the ambulance has completed.
  19. Furnish the details if any dispensary exist, what are the veterinary facilities and first aid box or medicines and medical equipment available, give details.
  20. To submit the Animal Verification Certificate certified by the Animal Husbandry Department 
  21. To submit the undertaking for the Ambulance grant - Annexure III
