Documents Required

AWOs under ABC:

The following documents marked (ü) under ABC scheme should be sent as annexure in support of your proposal: -

  1. Application in prescribed proforma
  2. Detailed proposal and its justifications alongwith gist of activities undertaken and assets required;
  3. Details of area to be covered for this programme; total dog population in this area duly certified by the concerned municipal corporation (s); method adopted for carrying out dog census and how & in what time the organization will complete the programme in this area;
  4. Photocopy of Registration;
  5. Memorandum of Association showing ‘animal welfare’ one of its objectives;
  6. The estimated cost of proposed immunization and sterilization of dogs along with number of dogs and the area to be covered;
  7. List of Governing Body/ Monitory Committee Member list;
  8. Audited Accounts of last three years duly certified by a Chartered Account i.e., (i) Audit Report, (ii) Balance Sheet, (iii) Receipt & Payment Account and (iv) Income & Expenditure Account;
  9. Details of Infrastructure viz. Operation Theatre, Kennels (with their capacity), Vet. Surgeon, Vehicle, man-power etc., available to implement this project;
  10. Details of activities undertaken in the field of Animal Welfare both in physical and financial terms during last three years;
  11. Details of financial assistance, if any, received from any other agency for this purpose;
  12. Details of collaborative arrangement that exists between the concerned Municipal Corporation (s) and the NGO to implement the project and a copy of Memorandum of Understanding entered into with every concerned municipal corporation;
  13. Furnish the photocopies of the Animal intake/out put register daily basis (date wise) duly signed by the Commissioner.
  14. Furnish the surgery register daily basis (date wise) duly signed by the local body official & Vety.Surgeon/Vety.Doctor with registration number.

The following documents for ABC programme is required by the Board.

Local Body:

  1. The Municipality if collaborated with any AWO/NGO for carrying out the ABC operations; an Original document of Memorandum of Understanding (MoU) duly signed by the Municipality & AWO to be furnished.
  2. Furnish the infrastructure/facilities availability in the Municipality/AWO (Shelter, Dispensary, Vety.Surgeon, Para Vets, Vety.Staff, animal handlers, medical equipment, Autoclave, supply of dogs, dog catcher, dog catching/relocating, existing system of drainage, ambulance etc.,
  3. Furnish the photocopies of the Animal intake/out put register daily basis (date wise) duly signed by the Commissioner.
  4. Furnish the surgery register daily basis (date wise) duly signed by the local body official/Vety.Surgeon/Vety.Doctor with registration number..
  5. Detailed proposal and its justification along with gist of activities undertaken and assets required.
  6. Details of area to be covered for this programme; total dog population in this area duly certified by the concerned Municipal corporation (s); method adopted for carrying out dog census and how & in what time the organization will complete the programme in this area;
  7. Details of financial assistance, if any, received from any other agency for this purpose
  8. Furnish the Acceptance of 50% fund borne by the Municipality.
  9. Furnish the Managing Monitory committee member list.
  10. Furnish the photographs and photocopy of the activities of the AWO of ABC maintained separately by the municipality.
  11. Furnish the Sterilization Certificate with month wise breakup detail for ABC operation duly certified by the local body officials.
